More BlockReader work
This includes some minor fixes to the BlockReaderCLI, and two new
BlockReader types
* MappedBlockReader - Apply a function to each block, transforming it.
* FlatMappedBlockReader - Apply a function to each block, expanding it
into one or more blocks.
